DESCRIPTION: \n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nPublic Health Care System and Local Health Plan Collaboration:\nPartnering to Improve Hospital Discharge and Follow-up\n\n\n\n\n\n\n\n\n\n\n\n\n\n&nbsp;\n\n\n\nThe California Health Care Safety Net Institute (SNI) and the Local Health Plans of California (LHPC) Institute are pleased to invite you to a convening of public health care system and Medi-Cal managed care plan leaders.\n\nThe transition from the hospital to a new care setting is a tenuous time when patients are at increased risk of complications and readmission. Both public health care systems and managed care plans play an important role in safely discharging patients from the hospital and ensuring that they receive appropriate placement and follow-up care.\n\nAt this event, system and plan leaders will:\n\n\nExamine system and plan roles and responsibilities in the discharge process and identify effective staffing models for coordinating transitions across care settings\nLearn how systems and plans implement clinical workflows and data exchange processes to support safe transitions to home or to lower levels of care (e.g., skilled nursing facilities, recuperative care, assisted living, etc.)\nIdentify promising strategies to discharge patients with complex health and social needs, such as homelessness, severe mental illness, and dementia\nUnderstand how systems and plans are partnering to meet new requirements under CalAIM's Population Health Management Transitional Care Services\nStrengthen relationships with system and plan partners through joint planning efforts \n\n\n\nDetailed Agenda\n\nParticipants:\n\nFour people per organization (systems and plans, respectively) are invited to attend.\n\no Note that SNI can reimburse travel expenses for up to 4 people per public health care system.\no Systems and plans are encouraged to coordinate attendance prior to registering.\nIdeally, participants will have a decision-making role in efforts related to hospital discharge, placement in community settings, and follow-up care.\n\no This may include, but not be limited to: hospital chief medical officers, hospital medical directors, chief medical informatics officers, directors of care management/coordination, and physician leads for throughput/transitions of care\n\nTravel Reimbursement and Registration:\n(Note that SNI can only reimburse expenses for public healthcare system members)\nTravel reimbursement is available for 4 people per public health care system. For participants traveling less than 150 miles to the meeting venue, SNI will reimburse 4 participants per system up to $225 per attendee. For participants traveling more than 150 miles to the meeting venue, SNI will reimburse 4 participants per system up to $550 per attendee. Please see the Expense Form for additional information.\nWe have a small block of hotel rooms available, though same-day travel is highly encouraged whenever possible. If you require a hotel room, please contact Abby Gonzalez by October 31.\n\nThe deadline to register is October 29. Please notify SNI of cancellations at least 7 days prior to the convening.
